Main duties of the job

District and Community nurses visit people who are housebound of all ages 18+ in their own homes or residential homes. This is an excellent opportunity for nurses who have met their competencies at band 5 level. You will need to be self-motivated, an excellent communicator, and a team player with the drive and determination in case management.

Detailed job description and main responsibilities

MAIN DUTIES
Managing Care:
To have responsibility for the effective delivery of nursing care to patients as delegated by District Nurse Team Manager.
To undertake skilled nursing activities related to the assessment, planning, and evaluation of patient care.
Managing Resources:
To teach and supervise the clinical practice of team members and identify training needs.
Quality:
To participate in policy and standard setting, quality initiatives, and clinical audit.

Person specification

Education / Qualifications

  • Registered Nurse (RGN 1st level registration)
  • Evidence of CPD
  • Further postgraduate certification

Experience

  • Proven ability to meet work-based competencies in a primary care setting
  • Understanding of 6Cs (including compassionate care)

Skills

  • Excellent verbal and written communication
  • Clinical/Research based skills

Knowledge

  • Impact of NHS policy on community nursing
  • Understanding of Clinical Governance
