Overview

Location: Hospital in Portsmouth, UK

Salary: £36.28 per hour

Contract Type: Ongoing

Work Schedule: Full-Time / Part-Time

Your Role

  • Hospital Discharge Planning: Complete Care Act assessments and arrange safe, timely discharges through the Discharge to Assess (D2A) model.
  • Integrated Working: Collaborate with health professionals, therapists, housing officers, and others to coordinate discharge planning and aftercare.
  • Reablement Focus: Promote independence and recovery, aiming to minimise reliance on long-term care and avoid hospital readmission.
  • Safeguarding Adults: Respond to concerns in accordance with Section 42 of the Care Act and local safeguarding policies.
  • Mental Capacity & Advocacy: Conduct Mental Capacity Assessments and Best Interest decisions; ensure access to advocacy when needed.
  • Care Reviews: Monitor and evaluate care packages for effectiveness, quality, and value for money.
  • Documentation: Maintain accurate records in line with legal and local authority standards using electronic case management systems.

What We're Looking For

Qualifications & Registration

  • Social Work degree or equivalent
  • Registered with Social Work England

Experience

  • Prior experience in adult social care, ideally in hospital discharge, intermediate care, or reablement
  • Solid understanding of the Care Act 2014, Mental Capacity Act, and NHS Discharge to Assess processes

Skills

  • Confident working in a multidisciplinary team
  • Strong assessment, planning, and time-management abilities
  • Experience with complex decision-making and working under pressure
  • Knowledge of Continuing Health Care (CHC) and NHS-funded care pathways
  • Familiarity with Mosaic or similar case management systems
