Locum Occupational therapist – Admission Avoidance Team (NHS)

We are seeking an experienced and motivated Locum Occupational therapist work within the NHS. This role is vital in supporting patients to remain safely at home, preventing unnecessary hospital admissions, and ensuring timely, person-centred care within the community.

Contract: Locum position, flexible hours available
Location: NHS Trust (details on request)
Start date: ASAP
Key responsibilities include:
• Rapid assessment and intervention for patients at risk of hospital admission
• Working as part of a dynamic multidisciplinary team (MDT)
• Delivering high-quality physiotherapy input in both acute and community settings
• Supporting patients and carers with rehabilitation, self-management, and safe discharge planning
Requirements:
• HCPC registration
• Proven NHS experience in acute, community, or admission avoidance services
• Strong clinical decision-making and communication skills
• Ability to work autonomously and as part of an MDT
